Caerphilly County Borough Council is seeking an experienced and dedicated Social Worker to join our 16 Plus Team. This is a fantastic opportunity to work with young people transitioning into adulthood, ensuring they receive the right support to achieve independence and stability.
You will play a key role in supporting care‑experienced young people, providing high‑quality assessment, care planning, and intervention. The role involves working closely with multi‑agency partners, advocating for young people, and ensuring they have access to education, training, housing, and emotional well‑being support.
Qualifications: Degree in Social Work and registration with Social Care Wales.
Experience: Experience working with care‑experienced young people, safeguarding, and care planning.
Skills: Strong assessment, advocacy, and communication skills.
Knowledge: A sound understanding of legislation, policies, and best practices related to looked‑after children and care leavers.
Other Requirements: Ability to work effectively as part of a multi‑agency team and support young people through complex transitions.
