A public health service in Ely is seeking a part-time Social Worker for the Learning Disability Partnership Team. This role offers a unique hybrid working model, allowing for flexible office and home working. We have an exciting vacancy in the Learning Disability Partnership Team for a part‑time Social Worker. The role is based at Princess of Wales Hospital in Ely and offers a hybrid model of practice (office up to three days a week with home working).
You will undertake Social Care and Mental Capacity Act assessments, complete Safeguarding and care and support planning, and prepare Community deprivation of liberty applications to the Court of Protection. You will also work in partnership with adults, their informal carers, and a multidisciplinary team of health and social care professionals.
The Cambridgeshire Learning Disability Partnership (LDP) brings together specialist health and social care services for people with a learning disability. The LDP is now working to a hybrid model with office‑based work up to three days a week.
