What you will do:

Conduct comprehensive assessments of individuals referred to the Adult Mental Health team.

• candidate will need to be an experienced social worker with a mental health background and LAS Community based and in the office 3 days a week

Collaborate with multi-disciplinary teams to develop and implement care plans tailored to each individual’s needs.

Provide one-on-one support and counselling to service users to promote mental health and well-being.

Advocate for service users, ensuring they have access to the resources and services they require.

Maintain accurate and up-to-date records in line with legal and professional standards.
