Contract Type: Permanent, Full-Time

This role is subject to references (2 references, with 3 years’ history where possible), social work qualification and registration with Social Work England, casual car documentation, and an Enhanced DBS check as part of pre-employment conditions. If you drive, you must have a full valid driving licence and provide your own car for work as well as appropriate documentation. A casual car mileage allowance applies where journeys are part of the role.

About the Team and Role

Due to expansion, The Preparing for Adulthood (PfA) Team has opportunities to join the team. The Preparing for Adulthood Team has been reconfigured into 3 teams within one Service, aligned with Children’s Services to support partnership and seamless transition to adulthood. The teams are:

  • Preparing for Adulthood: Living at Home
  • Preparing for Adulthood: Leaving Care
  • Preparing for Adulthood: Complex

We start working directly with young people at 16 (and with schools from age 14). The goal is to support transition to adulthood for young people who may have learning disabilities, learning difficulties, autism, ADHD, physical disabilities, health needs, complex trauma, or attachment needs. No day is the same.

We are person-centred and aim to prevent, reduce and delay the need for long-term services, while providing right-fit support with the young person and their circle to help achieve their goals. The team works across children and adults’ services, offering opportunities to shape future services.

Manchester has invested in Person Centred Planners to work with young people, carers and social workers to develop personalised transition plans and support young people to achieve them.

We seek enthusiastic and compassionate people who champion the rights and needs of young people with SEND, ensuring their voices are heard. Creative thinking on how to support young people to achieve their goals is encouraged. We are committed to strong induction and staff development. The role is based in Etrop Court, Wythenshawe, with hybrid working (35 hours per week between office locations and home). The role supports adult teams across the city and plays a part in life planning as young people become adults.

Key Responsibilities

The Social Work Team Manager leads the Neighbourhood team to enable population health and wellbeing, ensuring co-produced neighbourhood initiatives. The post holder collaborates with professional leads across professions and organisations to deliver care and support priorities for the city.

Responsibilities include:

  • Provide strong management direction and motivate team members through performance management and coordination to meet service priorities.
  • Provide professional consultation, support and guidance to assist decision making and case management decisions.
  • Manage safeguarding risk and quality assurance within an agreed framework, policies and statutory responsibilities in a multi-professional environment.
  • Collaborate with stakeholders to gather data to identify care needs and priorities and develop sustainable, customer-focused service and implementation plans.
  • Identify and support change and improvements in service provision to improve care and outcomes for Manchester residents.
  • Monitor budgets in line with City Council policies to achieve financial targets and forecast resource requirements.
  • Coordinate resources to ensure joined-up communication and efficiencies.
  • Demonstrate commitment to continuous self-development and service improvement.
  • Advance equality and ensure diversity is valued in employment, service delivery and communications.
