What you will do

• Conduct complex Occupational Therapy assessments for service users with increased needs, particularly within the crucial first 12 weeks post-discharge.
• Lead on care package optimisation, aiming to reduce double-handed care where suitable.
• Manage and support Occupational Therapy Assistants, focusing on case delegation and supervision.
• Triage referrals from various hospital and community settings, ensuring urgent manual handling and equipment needs are promptly addressed.
• Collaborate with NHS and Adult Social Care teams to bridge service gaps and guarantee continuity of care.
• Offer expert input on equipment provision beyond the NHS Therapy team’s remit and support discharge planning from care homes and out-of-area cases.
• Contribute to service development by enhancing data capture workflows and facilitating training via the Care Academy.
