Social Care Assessor (4610), Lewis & Harris
37 hours per week, permanent post
£35,291 - £38,127 per annum, inclusive of £2,997 Distant Island Allowance

As a member of the Assessment and Care Management Team the main purpose of the post is to maximise the independence and safety of adults and older people who have care needs. This will be done by undertaking assessments of need using the appropriate assessment tools and undertaking reviews of care arrangements. You will assess and support service users in need, to develop appropriate care packages to meet the service user/carers preferred outcome within specified resources or by enabling the service user to access community resources and to review care arrangements.

Duties will include assessing and reviewing service users with particular care needs such as frail elderly, adults with mental health, physical disabilities, alcohol/drug misuse, dementia, terminally ill and chronic sick; following assessments, to identify whether or not service users meet the eligibility criteria for services and to communicate this to the service user/carer; undertake risk assessments and reviews as allocated by the Senior Social Worker/Team Leader; and working with colleagues and other external agencies in support of the assessment process for service users and carers and ensure the care plan is tailored to the individual outcome.

You must have a good working knowledge of community care legislation along with an understanding of community care policy and developments. It is essential you have excellent verbal and communication skills. You must have the ability to work collaboratively with other agencies/organisations.

It is essential you hold a professional social care or health qualification e.g. HNC/SVQ Level 3 and/or the ability to work towards the registration requirements of the SSSC (if required).

You must hold a full UK, unrestricted driving licence.

This post is subject to membership of the Protecting Vulnerable Groups (PVG) Scheme as a legal requirement of a regulated role with Adults (Protecting Vulnerable Groups (PVG) scheme - mygov.scot).

Salary for the above post is inclusive of Distant Islands Allowance. Appointment to the post will be on the first point of grade.
