Weare offering an exciting opportunity to join the Band 6 workforce on J34within the Speciality and Integrated Medicine Clinical Service Unit (CSU). Weare looking for enthusiastic registered nurses to join our friendly,developing senior nursing team.J34is a 28 bedded ward, specialising in Older Adult Care for patients who areMedically Optimised for Discharge, accommodating both male and femalepatients. Successful candidates will be responsiblefor leading the staff and working in close collaboration with the MDT. Therole is to provide and be responsible for the assessment, planning,implementation, evaluation of individualised programmes of care and to workwith the discharge team to facilitate safe discharge of patient. Youwill deliver care without supervision and take responsibility as the primecare provider for a group of patients and at all times promote confidence inour commitment to provide excellent compassionate patient care. Apassion for working with our patients, strong leadership and performancemanagement is an integral aspect of this band 6 role with patient experienceand patient safety underpinning all aspects of the ward running efficientlyand effectively. Youwill be responsible for the nursing assessments as well as the operationalrunning of the ward and all it encompasses. Youwill need to provide assurance that standards of care are met and sustainedat all times.

Communicatewith patients and carers to ensure they have access to appropriateinformation.
Maintainand deliver compassionate patient focused care in accordance with LTHTguidelines, policies, values and behaviours. Provide assessment, planning,implementation and evaluation of nursing care for patients, ensuringdocumentation standards are upheld in accordance with NMC and LTHTguidelines, whether written or computerised.
Promote and maintain positive relationshipsbetween all staff involved in the care of patients.
Promoteevidence-based practice.
Participatein the audit and evaluation of clinical practice in the interests ofpromoting and improving quality care through action on findings.
Undertake duties on other wards ordepartments as and when required by service demands or patient need.
Act in such a way that always safeguards thehealth and wellbeing of children and vulnerable adults. Is familiar with andadheres to, the LTHT safeguarding policies.
Contribute to the promotion of a positiveclinical learning environment.
Keep up to date with developments, advancesand research in individualised patient care.Participate in research and or educationprogrammes relating to professional nursing issues and practice.Inthe absence of the Senior Charge Nurse take charge of the ward or departmentand co-ordinate ward or department activities.
