What You Will Do:

Conduct needs assessments for adults with immediate or urgent social care requirements, developing care plans to promote independence.

Provide short-term interventions designed to address urgent needs and facilitate long-term autonomy for service users.

Collaborate with multi-disciplinary teams, including healthcare professionals, community services, and external agencies, to provide holistic support.

Lead safeguarding efforts, ensuring the protection of vulnerable adults and managing identified risks in accordance with safeguarding protocols.

Offer crisis intervention to individuals facing sudden life changes, providing immediate, practical solutions to resolve issues swiftly.

Supervise and mentor junior team members, providing guidance and fostering an environment of professional growth and excellence.

Ensure compliance with relevant social work legislation (e.g., the Care Act 2014), maintaining accurate and up-to-date case records in line with local authority standards.

What We Are Looking For:

A qualified social worker registered with Social Work England.

A minimum of two years’ post-qualification experience in adult services or a similar setting.

Strong understanding of adult social care legislation, including the Care Act 2014, safeguarding protocols, and relevant policies.

Excellent communication skills, with the ability to build rapport and work collaboratively within a multi-disciplinary team.

Strong organisational skills and the ability to manage a diverse caseload independently, ensuring that care plans are effectively implemented and reviewed.

Crisis management experience and the ability to deliver practical solutions in high-pressure situations.
