JOB PURPOSE:
To work as a proactive and flexible member of thePhyllis Tuckwell (PT) multidisciplinary team, supporting patients, theirfamilies and carers as they become known to Phyllis Tuckwells inpatient andcommunity teams, and developing a seamless approach in response their needs.
The is a leadership role with responsibility toensure coordinated high-quality care for patients. It involves holisticassessment and identification of patient needs, decision making and careplanning alongside other MDT colleagues at PT and community partners. It requires implementation of a managementplan and evaluation, to ensure that responsive high-quality nursing care and support isavailable for patients, their families and carers.
Therole entails having clinical and symptom management knowledge, communicationand psychological skills, leadership and people management skills, as well as apassion to drive forward professionalstandards, patient safety and practice development.
RESPONSIBILITIES:
1.0Clinical
1.1To assess, plan, co-ordinate,implement and evaluate palliative care interventions based on an agreed plan ofcare that supports quality and timely care delivered to the patient and familyor carer.
1.2To be proactivein response to patients physical, psychological, social and spiritual needsensuring the highest of standards in nursing care.
1.3To ensure that careis centred around the person, family and their carer and that effectiveness,clinical safety and a joint approach with colleagues underpins PhyllisTuckwells practice.
1.4To act as aresource and support to colleagues involved in providing palliative carethrough giving advice, information, support and education.
1.5To work alongsidePT multi-professional team to ensure high standards are maintained andconstantly re-evaluated, to provide high quality individualised care.
1.6To liaise with otherhealthcare professionals to ensure effective continuity of care irrespective ofthe care setting.
1.7To manageworkload of patients under the supervision and direction of the
keyworker, ensuring patient safety and the principles of clinical governance areat the centre of all care that is delivered.
1.8 To provide optimal symptom management, psychologicaland emotional supporttopatients, their families and carers.
1.9 To assess the patients changing needswithin own knowledge and scope, liaising with, or referring to, other PTspecialists or external partners to support patient or family needs asappropriate, or for end-of-life care at home, or in the In-Patient Unit.
1.10 To takeon, where appropriate, responsibilities for a specific clinical area, proactivelyseeking to improve knowledge and practice in the specified area and disseminatingbest practice.
1.11 To work with nursing colleagues to assesstheir clinical competencies as appropriate, providing training andsupport.
1.12 To attend and contribute to clinicalmultidisciplinary meetings.
2.0Professional
2.1To always contribute to team cohesionthrough ongoing supportive relationships and consideration of colleagues.
2.2To make autonomous decisions whilst acting inthe patients best interests and recognise own limitations.
2.3To work in accordance with NMC Code ofPractice.
2.4To support nursing colleagues with theirrevalidation as appropriate, and with reflective discussions, as a delegatedresponsibility by the service manager.
2.5To be mindful of ownprofessional behaviours, conduct and practice when working as part of a multidisciplinaryteam or when giving talks representing PT.
2.6To ensure patientrecords are properly maintained in accordance with NMC guidelines and PT policies.
2.7To develop anarea of expertise and act as a resource for the multi-professional PT team.
2.8To maintainappropriate professional links with other health and social care agencies.
2.9To identify, andwork to mitigate risks, and report accidents or incidents in a timely way,ensuring appropriate action and the optimum safety and wellbeing of patients,their families and colleagues.
2.10To understand and adhere to the PT MedicineManagement Policy in relation to the correct handling, usage, andadministration of all medicines.
2.11To provide integrated clinical support tothe relevant clinical teams within PT and to external health care colleagues.
3.0 Leadership and Management
3.1Provide Leadership by example.
3.2Lead a team and to take responsibility forbeing the nurse in charge on a shift as required.
3.3Support the professional development ofcolleagues and champion areas of practice to support high quality nursing care.
3.4To undertake 1:1 meetings and annualappraisals with staff as delegated by the service manager, support and buddynew staff during their induction, and report any concerns as appropriate.
3.5To be familiarwith guidelines and regulations relevant to the role and care setting, particularlythose relating to clinical and information governance, risk management, healthand safety at work, and ensure they are always met.
3.6To takeresponsibility for ensuring that individual clinical supervision is accessed.
3.7To ensure a safe working environment is maintained,in accordance with the PT Fire and Health and Safety Policies.
3.8To report any feedback, concernsand complaints from patients, their relatives or other advocates to the servicelead/manager and be accountable for actions taken.
3.9To adhere the PT Lone Working Policyand Procedure when working in the community.
4.0Training and Development

4.5To maintain a dynamic interest in all professional matters associatedwith palliative care and management, with a particular interest in currentresearch and development if they relate to nursing practices.
5.0 Research and Audit
5.1 To participate with research and auditwithin PT encouraging staff participation where appropriate
5.2 Totake responsibility for specific audits ensuring audits are completed in atimely manner, results communicated to the team, and actions put in place forongoing improvements in practice.
6.0Safeguarding,Equality, Diversity and Inclusion
6.1 Role holders are required to understandtheir responsibilities in the safeguarding context, ensuring that PhyllisTuckwell reduces the risk of harm or abuse of adults and children at risk.
