Senior Pharmacy Assistant - Respiratory Medicine

This is an exciting opportunity to join the Interstitial lung disease (ILD) pharmacy team at Guy's and St Thomas' Hospital and provide pharmacy support. This role involves supporting the team in administrative tasks. You will be required to manage and maintain databases in use by the ILD pharmacy team. You will liaise with ILD patients, healthcare professionals and homecare providers as required to facilitate provision of homecare medication for ILD patients.

Main duties of the job

The postholder will support the ILD pharmacy team in the coordination and production of homecare prescriptions, ensuring relevant databases are maintained, liaising with patients, healthcare professionals and homecare providers as necessary to facilitate homecare for respiratory patients.

The post holder will support the ILD pharmacy team in communicating with primary care to gather information regarding patients' blood tests.

The post-holder will support the ILD Pharmacy and nurse specialist (CNS) teams to manage outpatient communication, liaise with referring Physicians and General Practitioners and deal with patient queries.

Job responsibilities

Communication

To work effectively as a member of the ILD Pharmacy team.

To liaise with ILD patients, healthcare professionals and homecare providers as required to facilitate provision of homecare medication for ILD patients.

To always communicate with colleagues in a polite and courteous manner.

To attend to personal callers and telephone requests in a courteous manner and to communicate effectively with patients, carers, nurses, and doctors.

To act as the first point of contact for ILD homecare patients, dealing with telephone calls and enquiries promptly and efficiently, providing immediate response and referring on to the appropriate member of staff as necessary. This may include personal enquiries from patients and their relatives, calls from homecare providers and trust staff.

To generate letters, post Pharmacist led ILD clinics and send these to patients, their GPs and referring centers and upload onto their electronic patient record.

To liaise with the ILD pharmacy team and ILD nursing team to organise and order medications for patients coming to the hospital for infusions.

Patient/customer care (both direct and indirect)

To regularly input data and maintain the databases, including inputting information for blueteq, which are in use by the ILD pharmacy and CNS teams.

To be responsible for reviewing the homecare companies requests spreadsheet for ILD patients for which prescriptions are required and when and prompt the ILD pharmacy and CNS teams when patients are overdue, or if a prescription was requested previously but not received yet.

To be responsible for printing prescriptions for ILD patients which have been requested by the homecare companies.

Liaise with GPs and secondary care providers to arrange blood test monitoring required for patients on anti-fibrotic medications and immunosuppressant drugs.

To take part in the review of working practices and procedures for the Pharmacy department. To propose changes and assist senior staff in their implementation.

To assist senior staff with project work when required.

To follow Trust and Departmental policies and procedures.

To update Pharmacy Department standard operating procedures relating to pharmacy activity data as required.

To use initiative and deal with matters independently where possible, but to refer non routine enquiries to a more senior member of staff as appropriate.

To ensure that all required information is readily available for Specialist Commissioning reporting.

Resource management

To be responsible for inputting information/patient activity onto computerised systems and ensure accurate retrieval and distribution of information.

To actively take part in the departments CPD program.

To attend relevant courses, training and business meetings as identified by your manager.

To take part in the training of Pharmacy and hospital staff as required.

Information management

To ensure that any information received from GPs or hospital doctors is uploaded to the electronic patient record as per agreed deadlines.

To ensure all patient and demographic details are checked and any amendments/corrections are carried out in an accurate and timely fashion.

Upload all relevant patient data into the appropriate database, as per agreed deadlines.

Actively contribute and participate in further development of electronic links with referring trusts and other hospital systems.

To be able to use pharmacy computer systems correctly, enter data accurately from prescriptions and ensure that data is stored and archived appropriately in accordance with Trust and departmental policies.

Research and development.

To provide administration support to the ILD and homecare teams with patient and administration audits for the development of new and revised processes in order improve service levels, quality of information and service levels.

Other duties

To undertake any other duties commensurate with the grade as requested.
