What you will do
  • Undertake comprehensive professional Continuing Healthcare and Funded Nurse Care assessments for adults with complex health needs.
  • Act as a Case Manager for clients receiving continuing healthcare funding, assessing and facilitating their care needs.
  • Communicate effectively with patients, families, and healthcare professionals, ensuring clear and accurate information transfer.
  • Develop, review, and manage care plans to meet patients’ needs, advocating for them and ensuring they receive optimal care.
  • Support and guide care providers, offering expert advice as necessary.
  • Participate in Continuing Healthcare panels, appeals processes, and service development initiatives.
  • Maintain accurate records and reports aligned with the NHS Continuing Healthcare and Funded Nursing Care frameworks.
