A local NHS Trust in Carlton Colville is seeking a qualified Mental Health Nurse, Social Worker, or Occupational Therapist to support people with dementia in care homes. The role includes care coordination, coaching staff, and promoting the transition from inpatient to community settings. Candidates should have experience in dementia care and a collaborative approach to support challenging behaviors.
There is an opportunity for a qualified Mental Health Nurse/Social Worker/Occupational Therapist to join the recently developed In-reach/Outreach service at Carlton Court Hospital. We are seeking motivated and enthusiastic individuals who have a passion for supporting people with dementia who are living in care homes and undergoing a transition from in-patient wards to Care Home settings. The role will also involve supporting and educating staff in the homes, case management and leading formulation meetings. You will work alongside registered and unregistered staff, plus a clinical psychologist.
We cover the Lowestoft, Waveney and Great Yarmouth area.
