A leading healthcare provider in Scotland is looking for a motivated health care support worker to join their Hospital at Home service. This role involves delivering high-quality patient care in home settings under the guidance of a multidisciplinary team. Candidates should have relevant experience in community or acute settings and possess a valid driving license. This position is pivotal for ensuring safe and effective patient care at home, contributing to the overall health of the community.

This is an exciting opportunity for an enthusiastic and motivated health care support worker to work within the developing Hospital at home service within Renfrewshire/East Renfrewshire. The post holders will be integral to the delivery of care to patients admitted to the hospital at home virtual ward. They will work under the direction and supervision of the registered MDT within the team and support the delivery of safe, high quality of care in the person’s home. Car driver necessary.
Candidate should be able to demonstrate relevant experience either within community or acute settings. The post holders will be based within the RAH within the MDT hospital at home team that works alongside acute teams and flow navigation plus to admit patients with frailty presenting with acute changes to hospital at home virtual ward and provide acute level care within the person home or homely environment. Car driving licence required.
