A leading health service provider in the UK is looking for a Discharge Coordinator to support patient flow in rehabilitation units. We are seeking aDischarge Coordinator to support patient flow across our Bedded Rehabilitation Units and help ensure safe, timely and well-planned discharges.
This role is key to supporting the patient journey from admission through to discharge, with early identification of needs, proactive planning, and close collaboration with multidisciplinary teams and partner organisations.
As a Discharge Coordinator, you will take a proactive approach to discharge planning from the point of admission. You will lead early conversations with patients and families to set clear expectations of the service, while identifying and addressing any immediate or potential barriers to discharge.
Your work will directly support the reduction of length of stay and the development of robust, efficient discharge processes within our rehabilitation units.
You will act as a central point of contact for all internal and external discharge-related enquiries and play a vital role in coordinating communication across services.
The role is primarily based within one rehabilitation unit, however you will be part of a wider team of Discharge Coordinators working across five services (four sites). You will be line managed by the Rehabilitation Units' Flow Lead and will work closely with:
