Job Overview

Are you a band 6 or an experienced band 5 looking for an exciting new opportunity? We are seeking a skilled, motivated, and dynamic Nurse with a passion for high quality care and service development.

Key Responsibilities
  • Manage your own clinical caseload of community patients.
  • Provide care within the Rehabilitation And Frailty Team (RAFT), delivering daily input for up to 21 days at the patient’s own home between 8am and 8pm, with an extended care element from 7am to 10pm.
  • Support HCA/rehabilitation assistants with training, supervision, and joint sessions.
  • Coordinate with other service providers to respond to changing daily needs, aiming to avoid admission where safe and supplying onward referrals to other community services.
  • Assist patients across a wide range of conditions – neurological, orthopaedics, falls, frail, end of life, admission avoidance, long‑term conditions and musculoskeletal.
  • Work occasional weekends and bank holidays on a rota basis; be prepared to provide service on a seven‑day work schedule.
  • Act as a registered nurse (RN12‑NMC) leading nursing care for a designated caseload and delivering intermediate and primary care services.
  • Maintain professional and legal accountability for all aspects of care within clinical areas.
Requirements and Eligibility
  • Registered Nurse with an NMC number (RN12‑NMC) at Band 5 or 6.
  • Significant intermediate and community care experience.
  • Ability to manage and support peers, work independently, and act as an effective team member.
  • Car driving licence – car driver essential.
  • Willingness to work 7 days a week, including occasional weekends and bank holidays.
  • Excellent communication skills and ability to liaise with health, social care professionals and other agencies.
Onboarding and Induction

All new starters will join the Trust on the first Monday of each month and will undertake a comprehensive induction lasting up to two weeks at our head office in Rainham, Essex. The induction includes mandatory training, system training, equipment allocation and drop‑in sessions focusing on engagement, health and wellbeing. New starters will also have the opportunity to meet the executive team, senior managers and attend sessions with the senior management team.

Probationary Period

This post will be subject to a probationary period; however, internal applicants are exempt unless they are already part‑way through a probationary period or are current bank members of staff.

Remuneration

The post includes a High‑Cost Area Supplement of 5 % of the basic salary (minimum £1,303 and maximum £2,198).
