A healthcare organization in Northallerton is seeking a Senior Community Mental Health Practitioner to join their multi-disciplinary team. The role involves assessing and coordinating care for individuals with complex mental health needs, providing evidence-based interventions, and mentoring junior staff. Ideal candidates should have significant experience in mental health care, strong communication skills, and a commitment to collaborative work with various agencies. Competitive salary ranging from £38,682 to £46,580 per annum, pro rata, is offered for this position.
This is an exciting opportunity to work as part of a community based multi-disciplinary team based in North Moor House, Northallerton.
As a senior practitioner you will have a genuine interest in collaborative working with people of varying ages and complex needs, including service users, Carers, families and our partners including statutory and voluntary agencies.
The successful candidate will participate in a range of interventions including triage, complex assessments and risk management and deliver evidence based psychological well being interventions (training will be provided) to support a purposeful service user journey whilst with the team.
The successful candidate may also be required to provide ongoing coaching support, clinical and caseload supervision to junior colleagues and wider members of the MDT.
Applicants must have 18 months post qualifying experience including sign off of appropriate preceptorship or competency framework.

As a core member of the team, you will also be responsible for your own caseload, providing assessments, care co-ordination and delivering intervention in the community with people who have complex needs ranging from mild to moderate level of need to severe and enduring level of need and complexity.
You will demonstrate excellent assessment and analysis skills, interpersonal, communication and team working skills. You will need to be committed to working collaboratively with a range of statutory and third sector agencies to promote holistic wellbeing and recovery focused care.

You will support the team's improvement activities for co-creation, aiming to deliver care that is highly personalised to each individual's needs and working closely with families, paid and unpaid carers.
