Qualifications

• Registered as a Pharmacist with the GPhC.
• Postgraduate Diploma in Clinical Pharmacy or equivalent (or working towards or willing to do).
• Membership in the Royal Pharmaceutical Society.

Responsibilities

• Provide a clinical pharmacy service to patients on the wards.
• Ensure patients understand how to take their medicines upon discharge.
• Support medicines safety and provide pharmaceutical advice.
