Qualifications

• A full UK driving licence (2 years) and access to a vehicle.
• Proven experience in multi-disciplinary working.

Responsibilities

• Coordinating referrals and liaising with hospitals and community teams.
• Supporting and mentoring staff, students, and volunteers.
• Conducting risk assessments to ensure safe living environments.
• Managing rotas and maintaining service continuity.

Education

SVQ Level 3 in Health and Social Care and PDA in Supervision (or equivalent)
