The Role You Play

As the Waiver Facilitator, you will be the primary point of contact for Pawnee clients who utilize or qualify for the State of Kansas Home and Community Based Services (HCBS) Severely and Emotionally Disturbed (SED) waiver. In this role, you will track services, handle clerical responsibilities, and ensure compliance with Kansas requirements. You’ll also become a subject matter expert on the program, offering support and consultation to clients, families, and staff.

The Day-to-Day
  • Monitor and track SED waiver services and outcomes to ensure State compliance and alignment with each client’s treatment pla
  • Submit waiver data and maintain accurate records, including Pawnee’s waiver spreadsheet and required State reports
  • Serve as the main contact for SED waiver referrals, assessing cases to determine eligibility and appropriate services
  • Coordinate and facilitate quarterly plan-of-care reviews with clients and families, managing scheduling and follow-up
  • Provide guidance and support to staff, community members, and clients on the waiver program, application process, and available services
