A friendly training practice in suburban Durham is seeking a fully qualified GP to work 4 sessions per week, combining face-to-face and phone consultations. The ideal candidate will possess excellent clinical and communication skills and be a collaborative team player. We are seeking an experienced General Practitioner to join our small, friendly team. We are offering a unique opportunity for a dedicated professional with at least 2 years\' experience in general practice.
The role carries considerable responsibility to ensure the highest standards of care for all our patients. The successful candidate will be involved in planning and decision making as part of the wider practice team.
There is an expectation that the post-holder complies with all clinical and information governance policies and procedures and any other policies and procedures necessary for the safe and effective discharge of all required clinical duties.
We would like the successful candidate to ideally have experience of Diabetes management or willingness to take this on as practice lead.
To provide support for the GP Partner and wider clinical team and supervise trainees.
We are a friendly GMS practice with a list size ofapproximately 5100
We have SystmOne as our Clinical System.
We have a supportive multidisciplinary team including GP Partner, 2 x salaried GPs, Nurse Practitioner, Practice Nurse, 2 x HCAs, pharmacist and pharmacy technician, Care Coordinator & administrative support staff and close links with PCN staff.
We work together as a team to deliver the best care possible for our patients.
We take a proactive approach to looking after our patients and focus on continuity of care and innovationin service delivery
We are a dedicated training practice, providing training and clinical support for 2 x GP Registrars and a Foundation L2 doctor. We also host 5th year medical students. We occasionally provide work experience for those considering GP as a career.
To undertake all duties and responsibilities associated with a General Practitioner working within Primary Care including:
Professional decision-making in relation to presenting problems.
Take part in audit activities when required.
Assessment of patient\'s healthcare needs
Consultations face to face, via telephone and home visits and dealing eConsults.
Triage calls and visits, checking and signing repeat prescriptions and dealing with queries, paperwork and correspondence in a timely manner.
Proactive screening of patients for risk factors and early signs of illness.
Disease management in line with protocols, developing care plans when required.
Lead in Diabetes management - preferably with experience or with a view to training
Providing health education as necessary
Attend and take part in MDT meetings with the wider team.
Compliance with all relevant clinical governance and practice policies and protocols.
Assisting with establishment of appropriate systems to manage long term conditions.
Completion of all relevant administrative duties required for successful delivery of service.
Record clear, detailed and timely consultation notes to agreed standards.
Demonstrate commitment to lifelong learning and audit and continuous development to ensure evidence based best practice.
Compile and issue acute and repeat prescriptions, prescribing in accordance with the formulary when appropriate.
Meet professional learning standards and CPD requirements.
Development of effective relationships with colleagues and the wider team which is very important for collaborative working.
Supervision and support for trainees during their placement at the practice.
Providing teaching and training session for trainees, as required.
