Main purpose of Job:
The Slough rough sleeping reduction service works as part of a multi-agency effort, liaising with the local authority, police and other agencies to address street homelessness, with a particular focus, upon those individuals entrenched in the cycle of repeat rough sleeping. The service will work with internal and external partners to identify, and respond to, reports of rough sleepers and maintain an accurate overview of the local picture. Provide advice and referrals to services which meet the customers individual needs e.g. health care, substance misuse services, housing and benefits. The overall aim is to enable people sleeping rough to address their needs, improve health and to find appropriate solutions to end their homelessness, also to encourage people to lead a more settled way of life, by providing support and appropriate referrals to a range of services to those, also at risk of rough sleeping.
Main Accountabilities:
In the Through Care Navigator role you will:
- Build and maintain productive relationships with internal and external stakeholders including local authorities, other homeless services, housing providers and any other external involved parties.
- Provide a client centred approach, offering support and advice to access health care for physical and/or mental health conditions, work and training opportunities and the relevant support for other support needs.
- Provide a client centred support plan for those in medium or long-term accommodation towards maintaining it available and addressing any signs of failure at an early stage to prevent them from returning to the streets or became unstable.
- Work very closely within the Outreach team, with Housing demand team and Temporary accommodation team in identifying the client group at risk of becoming homeless following potential breakdown of unstable accommodation or because of their undisclosed support needs.
- Visit clients at their temporary accommodation placement or sleeping site and identify a client tailored way of providing support at an early stage, with the aim of preventing homelessness.
- This role you will involve you meeting clients and professionals at different locations across Slough, and sometimes outside of the borough. This may include providing support to clients while they are on the streets, at local day centres and offices, in a range of supported hostels and in people’s homes.
- Expected hours would be Monday-Friday, 9-5, although there is a degree of flexibility that can be given, and additionally will have to joint working clients with the Outreach team. No Hybrid-working is applicable to this role.
- Manage a caseload of clients rough sleeping or at risk of rough sleeping in need of support to access services to address their support needs, drafting support plans enabling multi-agency interventions to prevent returning to rough sleeping.
- Provide cover to the Outreach team when and where necessary with outreach sessions either early morning or late at night
- Provide support to those in our off the streets accommodation when and where needed.
- To be responsible and accountable for, effective delivery of high-quality outreach services for rough sleepers and those placed under a duty in temporary accommodation.
- Undertake an assessment of needs and risk to ensure client’s individual needs are identified and met
- To respond to immediate needs and risks by engaging clients with relevant health and support services
- Enable clients to relocate or return to their home/area, involving specialist agencies where necessary
- Promote rights, choice, dignity and independence; Always working within appropriate professional boundaries
- This role will involve working outside of normal office hours, e.g. evenings and occasional, ‘night walks’ with other Agencies, in line with rough sleeper counts etc. including, conducting regular, targeted street outreach provisions
- Maximise the opportunities available for clients through development of partnerships with external/internal, providers
- To adhere to and contribute to maintaining safe systems of work and a safe environment, in particular, Lone Working procedures
- Maintain clear, concise and accurate records of work undertaken and all contacts relating to each client, including, carrying out all necessary administration in relation to the casework tasks
- Develop positive working relationships with clients in order to facilitate the best possible outcomes and to address issues of social isolation and exclusion, substances & alcohol misuse, mental health, physical health or any other supports needs.
- To complete and submit regular reports to Rough Sleeping Manager / Outreach coordinator as directed
- To carry out, within reason, any other duties necessary to the smooth running of the project and to undertake other duties as your line manager should require in keeping with the responsibility of the post
