Job description

RNLD – Nurse Assessor

£40 – 45k + £2400 car allowance

Office hours at home then travel for face to face assessments in the North West, with occasional national coverage.

Monday to Friday 9am to 5pm

Key Responsibilities of the RNLD – Nurse Assessor
  • Completing desk-based and face to face assessments of referred people for support.
  • Ensuring an APIE approach - Assessing, planning, implementing and evaluating.
  • To review reports and evidence outcome measures to inform clinical practice, business performance and quality measures.
  • To maintain continuous professional development and be able to produce your CPD folder upon request.
  • Able to identify and mitigate complex risks for a community setting.
  • To review pre-admission assessment in a timely and professional manner.
  • Producing clinically informed assessments with robust risk assessment and identifying risk mitigation measures.
  • Identify measures to ensure people can be enabled through positive support.
  • Attend internal and external MDT meetings to further inform the assessment and risk mitigation process.
  • Effective time management.
  • Record keeping, ensuring that accurate, safe and contemporaneous records are maintained.
