Release and Transfer Nurse Coordinator

We have an exciting opportunity for a Release and Transfer Nurse Coordinator to join our Primary Care team at HMP Wormwood Scrubs, a category B Prison located in London.

In this role you will provide the highest standards of healthcare to our patients, without compromising on efficiency and quality.

Hours

Monday to Friday: 07:30 - 15:30

Salary

This position is full time and you will receive an annual salary up to £55,086 per annum, depending on experience.

Main duties of the job
  • Follow and implement the Release and Transfer Pathway.
  • Oversee the management of assessments of health needs, planning, delivery and evaluation of care, including clinical risk assessments.
  • Manage face‑to‑face clinics with patients and serve as the experienced clinician for patients who may need additional support for their release or transfer.
  • Contribute to MDT discussions and work collaboratively with other healthcare professionals and prison colleagues for safer discharge planning.
  • Line‑manage other members of the Transfer & Release team and collaborate with release teams in the prison and with outside organisations.

Requirements

Job responsibilities

Our integrated approach follows the patient journey – known as New Models of Care. Teams will work together at each stage of the patient journey to support them as they arrive in the prison, deliver the care they need, promote good health and support resettlement, and respond to urgent and emergency needs.

At HMP Wormwood Scrubs, the Transfer and Release team is a subdivision of the Early Days in Custody Team (EDIC), and you will report to the EDIC Clinical Lead and the Modern Matron.

You will ensure that the Transfer & Release Team delivers the guiding principles and provides a supportive pathway for patients returning to the community or being transferred to another establishment.

Guiding Principles to Release and Transfer

  • Reduce unnecessary and confusing handoffs between healthcare services and ensure consistency in service transfer by initiating early discharge planning.
  • Ensure safe transfers between establishments that offer continuity of care and ongoing support.
  • Ensure patients’ goals and needs are considered in care planning prior to transfer, with appropriate actions for complex health care needs.
  • Remove delays for patients accessing and receiving necessary healthcare and medication during the transfer process.
  • Ensure released patients are pre‑registered with a GP in the community and have continuity of medication supplies.
  • Favour compassion and support, recognising health and care needs, strengths, and abilities to live an independent and fulfilling life outside secure estate.
  • Link patients into ongoing care and support, including adequate medication supply, via referral to community services nearest to accommodation.
  • Increase access to and uptake of healthcare or relevant support services for individuals with health vulnerabilities on release.
  • Work with community providers to maintain engagement in services after release.

Specific duties include following and implementing the Release and Transfer Pathway, overseeing assessments of health needs, managing face‑to‑face clinics, working closely with GP colleagues, requesting TTOs, identifying vulnerable patients, contributing to MDT discussions, and completing line‑management duties for the Transfer & Release team.

You will report performance data to the EDIC Lead and provide narrative around all performance areas within the team.

  • High levels of clinical expertise, decision-making, and judgement.
  • Knowledge of clinical audit, safeguarding, and statutory requirements.

  • Good IT skills and literacy.
  • Proactive problem‑solving and flexible attitude to working arrangements.
