West London NHS Trust is looking for an enthusiastic and dedicated Rehabilitation Assistant Practitioner to join our Hounslow Community Cardiorespiratory and Post-COVID Service. This is an exciting opportunity to make a real difference to patients living with long-term respiratory and cardiac conditions, as well as those recovering from Post-COVID Syndrome. You will be part of a specialist community team that delivers high-quality rehabilitation and self-management programmes across Hounslow, helping patients to manage breathlessness, fatigue, reduced mobility, and other symptoms that impact daily life.
Working alongside experienced physiotherapists, occupational therapists, nurses, and other members of the multidisciplinary team, you will play a key role in supporting patients through their rehabilitation journey. Your responsibilities will include carrying out delegated assessments and treatments, monitoring progress, reporting changes in condition, and supporting patients to use digital self-management tools such as the "Living with COVID" platform. You will also assist with home environment assessments, demonstrate safe use of mobility and rehabilitation equipment, and educate patients and carers on breathing techniques, pacing strategies, and energy conservation.
