Main duties of the job

Join a workplace that values compassion,integrity, respect, and teamwork. Working as one of the multidisciplinary specialistpalliative care teamin the provision of a co-ordinated andcomprehensive service to inpatients,their families and carers. This will be achieved through the provision of specialist palliative careinterventions based on comprehensiveholistic assessment, agreement of goal focused care planning, education andsupport to patients, families and carers.

Inconjunction with the Head of IPU, support the development of clinical practiceon the inpatient unit through the planning, implementation and evaluation ofservice development which meets the needs of the multidisciplinary team.

Job responsibilities

Main responsibilities:

1. Clinical Practice

1.1Working as part of theinpatient unit under the direction of the Clinical Sister / Head of IPU, Sisterand Senior Registered Nurse, facilitate the highest possible standards ofindividualised physical, psychological, social and spiritual nursing care forpatients, their relatives and carers prioritising need in line with availableresources.

1.2To work as a member ofthe team providing best possible outcomes for patients, families and carers,ensuring that the patients dignity is promoted and respected and that equalityand diversity are priorities of care.

1.3Undertake comprehensivespecialist palliative care nursing assessments, agreeing a plan of care forpatients and families based on clear goals intended to bring about effective,measurable outcomes.

1.4Work in partnershipwith other members of the multidisciplinary team to help patients achieve theirpreferred place of care.

1.5Provide advice and guidancerelated to symptom management, psychosocial and spiritual needs referringappropriately to relevant professionals or services.

1.6Carry out regularaudits ensuring that protocols, policies and integrated care pathways aredeveloped, implemented and evaluated and are consistent with national andinternational standards and current research; support changes in practice whichimprove clinical outcomes and meet the needs of patients, familiesandcarers.

1.7 Takeresponsibility for recognition and assessment of the educational anddevelopmental needs of colleagues,participating in the creation, delivery andevaluation of learning opportunitiesthat fulfill these requirements.

1.8 Provide clear explanations and educationas appropriate to ensure that patients, familiesand staff have the information they require in a timely fashion.

1.9 To act as a resource across the healtheconomy within this specialist area.

1.10 To participate in thedevelopment, review and audit of quality standards and contribute to the process ofcontinuous quality improvement monitoring, reporting to the Clinical Care & QualityAssurance Committee.

1.11 Ensurepatients and professionals have appropriate links to colleagues in the organisation ensuring continuity ofadvice and intervention through effective team working.

1.12 Regularlyattend multidisciplinary team meetings, proactively discussing patients, ensuringthe most appropriate plans of action are in place for individuals.

1.13 In the best interests ofpatients and families, proactively collaborate with a wide range of internaland external professionals developing effective working relationships andensuring effective communication channels are established between the partneragencies, patients and families.

1.14 Maintainfull, accurate and confidential clinical records, both in computerand written form, in accordance withmandatory legal requirements and theHospice standards for nursing documentation.

1.15 Challengeprofessional and organisational boundaries, identifying areas of skills, knowledge and development to providecontinuity and high-quality patient centred healthcare.

2. Administrative and Managerial

2.1Supervisethe work of junior team members and volunteers, providing a point of expertise and knowledge for complex patientproblems.

2.2 Providenursing support and mentorship to colleagues, ensuring clinical staff are preparedand supported in their roles.

2.3Supportthe team in the active development of Healthcare Assistantand volunteer roles.

2.4Participate in strategic service reviews and servicedevelopment initiatives as appropriate.

2.5Attend meetings pertinent to organisational and professionalresponsibilities.

2.6Maintain accurate and up-to-date statistics and utilise Hospicecomputerised systems to establishand monitor appropriate patient/client information. Inconjunctionwith the Senior Registered Nurses produce reports, analyses and documentation.

2.7Represent the Hospice on relevant external groups and beresponsive to and work withpolicy agendas as appropriate.

2.8Toparticipate in a rota that ensures provision of patient care 24 hours / 7 daysa weekas set by the Clinical Sister/ Head of IPU

3. Education, Training andProfessional Development

3.1Maintain own expertclinical knowledge and skills, including advanced communication skills, andachieve the educational standards set out in line with the skills, knowledge andcompetency level required of enhanced practitioners and by the role.

3.2To participate in theperformance review and appraisal process know as Your Snapshot. Identifypersonal development and training needs through clinical supervision and theperformance appraisal process, and undertake relevant training, including allstatutory and mandatory training and development.

3.3To participate in the inductionand ongoing training and development of all clinical staff, students onplacements and volunteers.

3.4 To participate in the Hospicesmultidisciplinary formal and informal teachingprogrammes for internal and external staff. This will includestudents undergoing courses or gaining experience in the Hospice.

3.5 Topromote a collegiate, innovative, creative and supportive learning environment that welcomes ongoing learning, change anddevelopment.

4. Quality, audit and research

4.1Keep abreast of research and audit activity to ensure thathis/her practice is appropriatelyupdated in line with latest guidance, research findings and organisational policies and procedures.

4.2Assess, report and manage risks in line with the Nursing andMidwifery Councils Code and theorganisations policies, and undertake to review practices and learn from experience.

4.3To comply with the Hospices policy and procedure in handlingany complaints.

4.4Contribute to and support the Clinical Governance framework ofthe organisation, and keep up todate with governance activity and latest policies and procedures.

4.5Initiate and/or participate in research and audit, and promotean awareness of and participatein the dissemination of research findings through clinical application to ensure enhanced patient/family care.

4.6Actively work to ratified clinical policies and procedures.

4.7Abide by all organisational policies and guidance in relationto confidentiality andinformationgovernance. Any disclosure ofconfidential information includingpersonalinformation kept on computer or other media made unlawfully outside the proper course of duty will be treated as aserious disciplinary offence.

4.8 Keep up to date with current legislation,professional practice issues and relevantdevelopments in palliative andbereavement care.

5. Communication

5.1 To promote close working relationshipsand good communication links with colleaguesat all levels throughout the Hospice, and maintain good communications with external contacts as appropriate.

5.2 Maintain effective links with educationaland palliative care organisations as appropriate.

6. Health and Safety

6.1 To take reasonable care of the health andsafety of yourself and others that may be affectedby what you do while at work, and to co-operate in the implementation of the Hospices health and safety policiesand procedures.

7. Public relations and marketing

7.1 To initiate and/or participate in theproduction of promotional material for distribution to potential stakeholders.

7.2 To support fundraising/volunteer eventsas appropriate.

8. General

8.1 Organiseand prioritise any competing demands of the role, completing duties as required within working hours.

8.2 Toconduct yourself at all times in a manner befitting the post and theprofessional dignity of the Hospice,and ensure that the Hospice is positively represented to the public and other stakeholders.

8.3To work as a member of the Hospice team to foster the teamscaring mission and to have acommitment to the cause of helping patients and their families with palliative care needs.
