Job description

Serving veterans on the Special Population HBPC/SCI PACT team, the RN uses the nursing process and evidence-based practice to collaborate with the core Interdisciplinary team, in a Home Based setting. The RN is responsible for all elements of the nursing process when providing and supervising direct patient care, administers medications and procedures, and influences care outcomes by collaborating with members of the interdisciplinary team. Travel required from duty station to veteran's home.

Duties

The Home Based Primary Care (HBPC) HBPC/ SCI/D RN/CM Outpatient Staff Registered Nurse (RN) is responsible for providing competent, evidence-based care to HBPC patients and oversight of licensed vocational nurses/nursing assistants, as appropriate to the setting.
