Registered Nurse - Emergency Frailty Unit- Ysbyty Ystrad Fawr
We are looking for an enthusiastic and motivated registered nurse with a passion for looking after acutely unwell, frail and older patients within a supportive acute geriatric unit called Emergency Frailty Unit (EFU). The unit is supported by 3 geriatricians experienced in the management of both acute medical problems as well as chronic geriatric problems. The unit is also supported by experienced therapy team, a discharge coordinator who works closely with community resource team and an advanced nurse practitioner.
The role offers experience in the management of various acute medical problems as well as chronic geriatric conditions.

We admit frail and older patients who need hospital admissions for -
1. Decompensated medical problems (for example CCF, AKI, COPD, decompensated neurological conditions like Parkinson's disease, Motor Neuron disease, Multiple Sclerosis and various types of dementias/delirium etc).
2. Functional decline due to various medical and psychological conditions needing efficient therapy input.
The principles of the unit are to take away the barriers of age discrimination and safe & efficient transfer of care to the community. .
The ability to speak Welsh is desirable for this role; Welsh and/or English speakers are equally welcome to apply.
