Job description

If this is the role for you, your responsibilities will include: Working as part of a multi-disciplinary team to aid the facilitation of a safe discharge and ongoing care of our enterally fed patients Directly liaising with Dietitians and Nutritional teams to manage the care and nutritional support of our patients within the community Responsible for the training of our enterally fed patients, families and carers Managing a patient caseload, providing ongoing support in the community for patients, carers and healthcare professionals Regular patient visits may include the care of Naso gastric tubes, gastrostomy tubes, pumps and advice Working within the NMC Code of Conduct and professional practice
