Job description

Want to make a real difference and have a truly rewarding career? Join 'Team Derbyshire Healthcare' and become part of a talented, compassionate and enthusiastic workforce committed to a vision of “making a positive difference in people's lives”.

We've been rated “good” overall by the CQC, with some outstanding services. The CQC commented at our most recent inspection on how our colleagues "treated patients with compassion and kindness" and "felt positive and proud about working for the trust."

We are passionate about making equality, diversity and inclusion part of our DNA. We take pride in our “People First Culture” which creates a workplace where everyone feels a genuine sense of belonging, difference is celebrated, and people are comfortable to bring their whole selves to work. We value the richness that comes from having colleagues that are fully representative of the local community.

We welcome people who identify as Black, Asian or from a Minority Ethnic background, protected characteristics, and with all backgrounds and experiences.

You’ll be playing a key part in introducing people into the Living Well Team, helping to identify their needs and designing person-centred, compassionate support that empowers them to live their best life. Collaboration is at the heart of this role. You’ll be working alongside colleagues across the community to ensure that the right support is delivered at the right time by the right person.

About the Teams

While the role is supported by the Older Adults CMHT, you’ll also be working closely with the Living Well Team, a multi‑agency group that places individuals right at the centre of their care. One of the most rewarding parts of this role is being part of a team that reaches out to people who may not have had access to mental health support before.

If you’re someone who cares deeply about helping others and wants to work in a team that values compassion, collaboration, and community, we’d love to hear from you! The successful candidate will need to have the ability to travel across the geographical area.

Responsibilities
  • Clinically responsible for supporting the delivery of a flexible, responsive local service, taking forward an Integrated Wellbeing Pathway in the designated locality, which effectively supports the delivery of the Community Mental Health Framework.
  • Working collaboratively with colleagues, key partners, and those within the local community to develop services to meet the mental health needs of the community.
  • Employing a wide range of skills, building on and developing their expertise and actively seeking advice and collaboration with issues which may not be readily within their sphere of practice. The requirements for this post are to remain within the guidelines for professional practice inclusive of HCPC/NMC requirements or equivalent professional body.
  • Undertaking comprehensive person-centred psychosocial assessments (conversations), risk assessments and formulation of presenting mental health needs.
  • Building therapeutic relationships, developing a plan of care with that person and assisting them through their difficult time by empowering and enabling them to take control over their health.
  • Supporting and motivating individuals to improve their mental health by helping them develop self‑care skills, build resilience and better utilise community resources. A key part of this developing role will be to make and maintain relationships with community partners including statutory organisations, Voluntary, Community and Social Enterprise (VCSE) organisations and the local communities who we serve.
  • Hearing and responding to the voice of lived experience is a key element for developing this service.

As an employer and sponsor licence holder, to be able to provide sponsorship to overseas nationals via the Skilled Worker route we must ensure that we adhere to the sponsorship requirements set by UK Visa's and Immigration. Please note that as the current salary threshold is £25,000 it is unlikely we will be able to offer sponsorship for this post as without 2 years previous NHS Service at Band 3 you will commence at the bottom of the pay band which currently does not meet the minimum salary requirement for sponsorship (bank and agency service is excluded). This would apply to anyone who has current time limited right to work in the UK and would require sponsorship in the near future commencing at the bottom of the band. For details on eligibility criteria for a Skilled Worker visa, please follow this link to the gov.uk website Skilled Worker visa: Overview - GOV.UK (www.gov.uk).

Benefits & working conditions
  • Commitment to flexible working where this is possible.
  • 27 days annual leave/year plus bank holidays, increasing to 29 days after 5 years & 33 days after 10 years' service.
  • Yearly appraisal and commitment to ongoing training.
  • Generous NHS pension scheme.
  • Good maternity, paternity and adoption benefits.
  • Health service discounts and online benefits.
  • Incremental pay progression.
  • Free confidential employee assistance programme 24/7.
  • Access to our LGBT+ network, BAME Network and Christian Network.
  • Health and wellbeing opportunities.
  • Structured learning and development opportunities, flexible working.

As a flexible working friendly organisation, we want to be sure that you can work in a way that is best for our service and for you. Speak to us about how we might be able to accommodate a flexible working arrangement whether that’s job share, part time or another flexible pattern.
