Job Title:Social Worker – Adults Locality Team
Location:Wolverhampton
Rate:£33.20 per hour

An exciting job role has recently become available at Wolverhampton City Council. They are looking for a new member for their Adults Locality Team.

What will your responsibilities be?

Conduct thorough assessments and create personalized care plans based on individuals’ needs. Provide professional social work, working alongside agencies, providers, and health colleagues to assess and provide appropriate interventions. Deliver a high quality and effective social work service that involves engaging with adults, carers, families, groups, and communities in a timely manner and within statutory guidelines. Carry out Care Act compliant conversations using a Three Conversation, strengths-based approach that determines need, plans, and delivers focused intervention to safeguard adults with care and support needs and promotes positive wellbeing.

Qualifications and Experiences:

  • Degree level or equivalent in Social Work
  • preference will be given to candidates with experience with Care Act conversations, duty, MCA’s, MDT meetings, and safeguarding
