Through early intervention, holistic assessment, and person-centred planning, you will ensure individuals receive the right support at the right time.

• Conduct holistic needs assessments and develop tailored support plans in line with the Care Act.
• Apply robust safeguarding knowledge to protect vulnerable adults and manage associated risks.
• Promote independence and wellbeing by connecting individuals with appropriate services and community resources.

• Maintain accurate, high-quality records using case management systems.
• Work collaboratively with health, housing, voluntary, and multi-disciplinary partners to deliver coordinated, effective support.
