Role Purpose

As a Prevention & Wellbeing Social Worker, you will play a vital part in promoting independence, preventing escalation of need, and supporting adults to live fulfilled lives. Through early intervention, holistic assessment, and person-centred planning, you will ensure individuals receive the right support at the right time.

What You Will Do
  • Conduct holistic needs assessments and develop tailored support plans in line with the Care Act.
  • Apply robust safeguarding knowledge to protect vulnerable adults and manage associated risks.
  • Promote independence and wellbeing by connecting individuals with appropriate services and community resources.
  • Advocate for carers’ rights, ensuring their needs are recognised and addressed within care planning.
  • Maintain accurate, high-quality records using case management systems.
  • Work collaboratively with health, housing, voluntary, and multi-disciplinary partners to deliver coordinated, effective support.
  • Social Work qualification with current professional registration.
  • Strong understanding of safeguarding legislation and best practice.
  • Experience using adult social care case management systems.
  • Excellent communication skills and a commitment to person-centred practice.
  • In-depth knowledge of the Care Act and its application in frontline practice.
