Locum Social Work Team Manager within the Home to Decide Team in Merton, UK, earning £44.50 per hour. This role supports the Home to Decide Team to help residents return home from hospital, fostering independence and reducing the need for residential care. Working under the supervision of the Team Manager, you will supervise social workers and staff, contributing to impactful social care assessments, support plans, and safeguarding practices. With the Resilience Service forming the umbrella for this and other teams, the role is pivotal in reducing hospital discharge delays and ensuring continuity of care.

What you will do
  • Provide support and supervision to social work staff and associated personnel in the Home to Decide Team, ensuring high standards are met in all aspects of social care.
  • Screen and allocate referrals effectively, maintaining the smooth operation of the team and ensuring clients receive appropriate and timely support.
  • Handle complex casework with confidence and professionalism, guiding staff through intricate cases with a focus on safeguarding and achieving the best outcomes for clients.
  • Participate in the Duty Rota, acting as the senior practitioner based in the office, with minimal home working to ensure consistency and availability for urgent needs.
  • Foster independence among residents, supporting them to achieve personal objectives and return to their previous level of autonomy.
