Job description
Overview

Dream Medical are looking for a Primary Care nurse to work within a large category C training/resettlement prison which can currently house up to 1,605 prisoners and may increase in size by a further 500. You'll be joining a friendly, vibrant team who are committed to delivering the highest possible standard of care.



Responsibilities


  • As a Primary Care Nurse, you can expect to be involved in everything from clinical triage to the evaluation and development of patient care and standards.

  • You'll carry out – and potentially develop – nurse-led clinics, reviewing patient histories, undertaking relevant examinations and tests, organising any necessary treatment and referring patients to other healthcare professionals when you need to.

  • Across everything you do, you'll maintain accurate records and provide the kind of helpful, empathetic support that will make every patient feel at ease.

  • We'll utilise the wellbeing model to put patient care and safety at the heart of everything we do through assessing prisoners across 5 domains of; physical and mental wellbeing, substance misuse, relationships and resettlement.

  • The patient and clinician complete a wellbeing wheel assessment to set goals and outcomes in partnership. The patient will complete the assessment and any needs identified will be met by our fully integrated service delivery team.
