Job Description – Practice Nurse

Job Summary We are seeking a dedicated and enthusiastic Practice Nurse to join our supportive primary care team. This is a flexible role, offered on a full-time or part-time basis, with a negotiable salary dependent on skills and experience. The successful candidate will play a key role in delivering high-quality nursing care to our patient population, working closely with GPs and other healthcare professionals.

Main Duties & Responsibilities

  • Provide chronic disease management (e.g. asthma, COPD, diabetes, hypertension).
  • Perform cervical cytology (smear tests), childhood and adult immunisations, and seasonal flu/COVID-19 vaccinations.
  • Conduct wound care, dressings, and phlebotomy as required.
  • Support patients in health promotion, lifestyle advice, and self-management of long-term conditions.
  • Triage and manage patient presentations appropriately, escalating to GPs or senior colleagues where needed.
  • Maintain accurate and timely clinical records in line with NMC standards.
  • Participate in infection control, safeguarding, and clinical governance activities.
  • Contribute to QOF, IIF, and other quality targets, supporting practice and PCN priorities.

About You

  • Registered Nurse (NMC) with experience in a primary care or community setting.
  • Confident in chronic disease management and preventative healthcare.
  • Competent in vaccinations, cervical screening, and phlebotomy (training can be provided if needed).
  • Excellent communication and teamwork skills, with a patient-centred approach.
  • Ability to work independently and as part of a multi-disciplinary team.
