Overview

We are looking to recruit a caring and motivated Practice Nurse who is committed to patient care and willing to further develop our dedicated nursing team. We are ideally looking for a Nurse with a very strong background in Diabetes care and General Practice Nursing.

The successful candidate will work collaboratively with our other Practice Nurses and HCA to treat our patients, maintain systems including infection control to meet CQC requirements and influence and implement policy and change in the Practice. This is a part-time position, ideally for 3 days per week. Salary is competitive, depending on experience, with access to NHS pension scheme.

Responsibilities

• Adhere to the NMC Code of Conduct and Scope of Professional Practice.
• Provide nursing advice and be a resource of best practice for the team.
• Assess, plan, implement, monitor and evaluate clinical work carried out to ensure effective care delivery in partnership with patients/carers. Promote and advise on healthy lifestyles.
• Carry out treatment room duties in line with competency (e.g., Suture/clip removal, basic and complex wound care, ECG, Phlebotomy, Urinalysis).
• Vaccination under patient group directions and ordering of vaccines, ensuring correct storage.
• Monitor and support the management of patients with long-term conditions/chronic diseases with a holistic, patient-centred approach.
• Assist with the cervical screening programme.
• Administer prescribed drug therapy and ensure safe custody of drugs.
• Assist with Warfarin monitoring.
• Assist in the provision of minor surgery, coil and implant fittings.
• Assist with co-ordination of Infection Control policy and audit.
• Provide training and supervision of junior nursing staff.
• Continue education/development to benefit service delivery and meet educational requirements.
• Ensure confidentiality at all times.
• Work with the nursing team to ensure clinical areas meet a satisfactory infection control level and remain adequately stocked with consumables.
• Other duties as reasonably required, consistent with the responsibility, training and scale of the post.

JOB PURPOSE

Topromote the mental, physical and emotional health of the Practice population

To assess the health needs of the Practice population

Toprovide and maintain a high standard of care for the Practice population

To be an effective member of the Primary Healthcare Team and to link with other organisations and disciplines in the development of services

DIMENSIONS

Clinical practice

To work collaboratively with other members of the nursing team to influence and implement policy and change in the Practice

To maintain an awareness of changing services needs within the scope of services in Primary Care, the Trust and Social Services

To be responsible for the effective use of resources within the sphere of responsibility

To ensure the maintenance of accurate and up to date manual and electronic records, as well as collecting and recording activity monitoring data, care aims and other contractual requirements

To undertake such other duties as may be reasonably required from time to time as are consistent with the responsibility and scale of the post
