Job description

We are looking for an enthusiastic and reliable Practice Nurse to support our practice team in delivering primary care service to ourdiverse patient population.

You will be working with a varied patient demographic and an experiencedMulti-disciplinary team.

Main duties of the job

The ideal candidate will have experience in the core competencies of practice nursing including wound care, child immunisation, independent and proactive chronic disease management (in particular diabeties) and cervical cytology. In addition, experience of management responsibilities would be beneficial, as there will be scope for the successful applicant to take on additional individual organisational and management responsibilities.

Job responsibilities

Job summary:

  • Helping the practice develop and deliver specialist primarynurse led services including chronic disease management. Make professionallyautonomous decisions and provide safe, evidence based, cost effective,individualised patient care
  • Providing assessment, screening and treatment services andhealth education advice
  • Cytology
  • Wound care
  • Travel Clinic
  • Asthma/Peak Flow Readings
  • Routine immunizations/Childhood immunisations
  • Requesting pathology tests, for example urine culture, swabs
  • Diabetic reviews
  • Other tasks will include:
  • Maintain general tidiness and cleanliness of nurses andtreatment rooms
  • Maintain stock of waiting room leaflets as instructed bypartners and Practice Manager
  • Ordering of vaccinations to maintain stock levels
  • Attend and participate in any Practice meetings whenrequired.
  • Following appropriate training, undertake the collection,storage and despatch of pathological specimens including intravenous bloodsamples, swabs, smears etc. and ensure the cleanliness of any equipment soused. Perform any investigatory procedures requested by the GPs.
  • Ensure accurate notes of all consultations and treatmentsare recorded in the patients notes on the clinical computer system asappropriate
  • Ensure accurate completion of all necessary documentationassociated with patient health care and registration with the practice
  • Attend and participate in practice meetings as required
  • Maintain a notice board in the waiting areadesignated for patient health care education

Qualifications
  • Qualified Nurse
  • Qualified in management of diabetes
Experience
  • Experience working within a nursing team with clinical and non-clinical colleagues providing an optimum level of care delivery to patients.
  • Experience of taking and maintaining responsibility for the nursing components of care.
