Responsibilities
- Provide skilled nursing care, including administering immunizations, wound care, and performing health screenings.
- Support patients in managing chronic conditions and developing personalized care plans.
- Promote public health through patient education, health promotion campaigns, and preventive care initiatives.
- Perform clinical assessments, including routine health checks, cervical smears, and diagnostic tests.
- Support the GP and other healthcare professionals with minor surgeries, investigations, and patient management.
- Maintain accurate records and documentation in compliance with clinical and legal standards.
- Engage with the community, identifying health trends and specific needs to tailor services accordingly.
- Prioritize creating a safe, inclusive, and respectful space for patients from all backgrounds.
Typical Day
- Conduct patient consultations and assessments, ensuring all people feel valued and supported.
- Manage chronic disease clinics, such as diabetes, asthma, and hypertension monitoring.
- Administer vaccinations, including flu, childhood immunizations, and travel vaccinations.
- Educate patients on diet, exercise, and lifestyle interventions to improve their overall health.
- Support emergency or same-day patient needs, such as wound dressing or urgent referrals.
- Collaborate with GPs, Healthcare Assistants, and administrative staff to provide holistic care.
- Keep up-to-date records of patient interactions using the practice's medical system.
- Actively engage with the team in meetings and contribute to quality improvement initiatives.
Qualifications
- Valid nursing qualification and registration with the Nursing and Midwifery Council (NMC).
- Previous experience in a primary care setting is desirable but not mandatory.
- Skills in administering immunizations and supporting patients with chronic conditions.
- Strong organizational, interpersonal, and communication skills.
