Position Summary

Population Health Management Analyst needed in Bexleyheath.

Rate: £24.03ph PAYE. This is a temporary role.

Responsibilities

The successful candidate will provide dedicated analytical capacity to embed Population Health Management (PHM) approaches across the division and the Integrated Care System (ICS) at place level.

The post holder will bridge public health intelligence and NHS data‑led service planning, enabling the design, targeting, and evaluation of interventions that reduce health inequalities, improve outcomes, and enhance patient experience. They will use existing datasets to support segmentation, forecasting, and equity audits, while preparing the system for future linked data capabilities.
