Job description
Overview

Harrow Rapid Response is a national leader in intermediate care that aims to bring the best of hospital care to a patient's home and therefore avoid unnecessary hospital attendances/admissions. Most referrals are from GPs, London Ambulance Service and Accident and Emergency departments. Attending individually and in pairs, we perform an initial holistic assessment at home and, in some cases, start a treatment plan within 2 hours of referral. The Team can utilise diagnostic equipment in the home and have access to hospital facilities for scans when required. We also perform services in A&E to avoid hospital admissions and facilitate safe discharges. Harrow Urgent Community Response Team aims to provide the same level of care you would expect as an inpatient, supported by real-time access to patient records in the community via laptops.

We are looking for a Physiotherapy Assistant who is passionate about patient care, eager to develop skills and push themselves beyond usual practice boundaries, and who has a real drive to ensure patients receive the best possible treatment. Excellent communication skills are essential.

Main duties of the job relate to the HUCRT/A&E service and the post holder will work as a therapy technician to support patients to regain and sustain their levels of function.

Responsibilities
  • Work as a therapy technician in HUCRT, supporting patients to regain and sustain their levels of function.
  • Assess patients, define therapy needs, carry out treatment, and maintain clinical records.
  • Undertake development of new skills, including nursing, physiotherapy and occupational therapy skills, and plan social care packages in agreement with social services. All skills are competency-led.
  • Collaborate closely with hospital colleagues and external agencies (Social Services, CCG, Housing Associations, GPs and voluntary organisations) and participate in meetings outside the Trust and share care plans and protocols with relevant agencies.
  • Work with patients, carers and colleagues in health and social services to ensure effective service planning, policy and delivery for the target patient group.
  • Be prepared to assess and treat patients with a wide range of complex physical, sensory, perceptual and cognitive problems within A&E and community settings; some inpatient assessments may also be required.
Qualifications and Skills

Education/Qualification

  • Technical qualification e.g. NVQ level 2/3 or equivalent core skills gained through work experience with patients in a hospital or community setting

  • Ability to teach practical skills to patients
  • Ability to work single-handedly with individuals/groups within a supportive environment
  • Understanding of professional conduct

  • Understanding of the patient's rehabilitation needs
