Overview

Nottingham West PCN covers 12 practices with a well-established clinical pharmacy team in practice as well as a pharmacy lead cardiovascular service, prescribing for patients in hypertension and lipids, across the PCN. This service won a PresQipp award and was shortlisted for the National General Practice awards.

We are a well-established, forward thinking PCN with a large and supportive clinical pharmacy team. Medicines Safety is at the heart of our team, working at PCN level to ensure high risk medicines are monitored regularly, along side innovative system one visualisations on drug monitoring.

We have a strong pharmacy technician team, led by our senior pharmacy technician. We have an exciting opportunity for a Pharmacy Technician to join our dynamic team. We are looking for someone who is enthusiastic about providing high quality care. Our Senior Pharmacy staff will support you to fulfil the requirements of the role and you will have access to the CPPE pathway in order to develop your skills. We are committed to further the development of Pharmacy Technicians within Primary Care.

Job responsibilities

JOB PURPOSE

Clinical Pharmacy in General Practice is part of a programme to develop a new model of care to deliver person-centred, coordinated care. The model aligns with national policy (Five Year Forward View and GP Forward View) to utilise pharmacy within primary care to help patients stay safe and well and reduce hospital admissions.

Pharmacy Technicians play an important role, complementing Clinical Pharmacists and other PCN team members. Technicians are not able to prescribe or make clinical decisions, and work under supervision to ensure effective and efficient use of medicines.

Technician responsibilities cover clinical, technical and administrative tasks. The role aims to maximise safe, cost-effective prescribing and improve patient care, including promotion of repeat dispensing and online ordering, minimising clinical risk and reducing wasted medicines.

The role also includes developing medicines optimisation understanding across practice teams and supporting NICE implementation and medicines optimisation initiatives.

KEY RESPONSIBILITIES

Clinical

  • Undertake patient facing and patient supporting roles to ensure effective medicines use, through shared decision-making conversations with patients.
  • Carry out medicines optimisation tasks including administration, supporting medication reviews and medicines reconciliation. Where required, work with patients to ensure effective medicine use.
  • Support medication reviews and medicines reconciliation for new care home patients and synchronising medicines for transfers between care settings, linking with local community pharmacies.
  • Support Clinical Pharmacist in Structure Medication Reviews (SMR) by organising necessary monitoring tests prior to SMR.
  • Provide guidance to address public health and social care needs, including lifestyle advice and tackling local health inequalities.
  • Manage shared care protocols and liaise with Clinical Pharmacists for complex patients.
  • Support antimicrobial stewardship to reduce inappropriate antibiotic prescribing locally.

Technical and administration

  • Support the PCN multidisciplinary team to ensure efficient medicines optimisation processes.
  • Implement efficient ordering and return processes and reduce medication waste.
  • Provide training on legal, safe handling of medicines, including Electronic Prescription Service (EPS).
  • Promotion of Electronic Repeat Dispensing (eRD) and online ordering.
  • Develop relationships with other pharmacy professionals and the multidisciplinary team to support integration across health and social care.
  • Support practice Reception Teams in streaming general prescription requests to enable review of clinically complex requests.
  • Support implementation of national prescribing policies and guidance within GP practices, care homes and other primary care settings.
  • Support PCN to deliver on QIPP agenda, QOF and locally commissioned enhanced services.
  • Support PCN in developing practice policies for CQC requirements.
