Job description

We are seeking an experienced Nurse Associate to join our innovative PCNComplex Wound Service. You will be a key member of our specialist wound careteam, delivering high-quality complex and post-operative wound managementacross North Shropshire. This role offers an exciting opportunity to workautonomously within your scope of practice while being supported by amultidisciplinary team.

Main duties of the job
  • Undertake comprehensive wound assessments using validated assessment tools
  • Develop, implement and evaluate individualised wound care plans
  • Perform complex wound dressings and treatments following PCN protocols
  • Monitor wound healing progress and adjust treatment plans accordingly
  • Provide post-operative wound care and suture/clip removal
  • Administer medications within scope of practice
  • Maintain accurate clinical records on EMIS
  • Participate in wound photography and measurement for monitoring purposes

Job responsibilities

KEY RESPONSIBILITIES

Clinical Duties

  • Undertake comprehensive wound assessments using validated assessment tools
  • Develop, implement and evaluate individualised wound care plans
  • Perform complex wound dressings and treatments following PCN protocols
  • Monitor wound healing progress and adjust treatment plans accordingly
  • Provide post-operative wound care and suture/clip removal
  • Administer medications within scope of practice
  • Maintain accurate clinical records on EMIS
  • Participate in wound photography and measurement for monitoring purposes
  • Deliver compassionate, patient-centred care to individuals with complex wounds
  • Provide health education on wound care, nutrition, and lifestyle factors
  • Support patients' self-management where appropriate
  • Identify and escalate concerns regarding wound deterioration or complications
  • Liaise with patients' families and carers regarding wound management

Professional

  • Work collaboratively with the Band 5 Nurse Lead and wider team
  • Supervise and support General Practice Assistants and Healthcare Assistants
  • Participate in clinical audits and quality improvement initiatives
  • Maintain professional registration and revalidation requirements
  • Attend mandatory training and wound care education sessions
  • Contribute to service development and protocol refinement

Service Delivery

  • Work across both East and West hub locations as required
  • Provide cover for enhanced access hours on a rota basis
  • Support the delivery of 20+ wound appointments daily
  • Ensure efficient clinic flow and appointment utilisation
  • Maintain clinical areas and equipment to required standards

PERSON SPECIFICATION

  • Registered Nurse Associate with current NMC registration
  • Minimum 2 years post-qualification experience
  • Experience in wound care management
  • Evidence of ongoing professional development

  • Proficient in IT systems including EMIS
  • Full UK driving licence and access to vehicle
  • Flexibility to work enhanced hours including evenings and weekends
