Job description

The Clinical Pharmacist post has been created to provide the leadership necessary to create a sustainable clinical pharmacy service for this PCN.

The clinical aspects outlined in the roleresponsibilities are intended to:

  • Support and improve prescribingand medication-related processes at a practice level
  • Improve the safety of prescribing,especially around repeat medication and transfers of care
  • Improve the quality ofprescribing, with a specific focus on National Service Specifications
  • Understand and address medication-relatedchallenges specific to this PCN
  • Enable Practices to optimizemedicines-related targets in QOF
Main duties of the job

The key responsibilities for the PCN pharmacist have beendetermined by NHS England and are described on page 20 of the Network Contract DirectedEnhanced Service Specifications1:

These are set out as follows:

a.Clinical pharmacists will work as part of amulti-disciplinary team in a patient facing role to clinically assess and treatpatients using their expert knowledge of medicines for specific disease areas.

b.They will be prescribers, or will be completingtraining to become prescribers, and will work with and alongside the generalpractice team. They will take responsibility for the care management ofpatients with chronic diseases and undertake clinical medication reviews toproactively manage people with complex polypharmacy, especially the elderly,people in care homes, those with multiple co-morbidities (in particular frailty,COPD and asthma) and people with learning disabilities or autism (via STOMP- StopOver Medication Programme).

c.They will provide specialist expertise in theuse of medicines whilst helping to address both the public health and socialcare needs of patients at the PCNs practices and to help in tacklinginequalities.

Person Specification
Qualifications
  • Degree in Pharmacy
  • Current registration with General Pharmaceutical Council, which must be maintained.
  • Member of Royal Pharmaceutical Society.
Experience
  • Knowledge of policy governing general practice and its operations.
  • Knowledge of financial drivers within general practice.
  • High level of clinical acumen, with the ability to plan, manage, monitor, advise and review medication and medical conditions.
  • Awareness of personal clinical limitations and ability to seek help when needed.
  • 2 + years experience as a practicing pharmacist post qualification.
  • Clinical pharmacy experience, ideally having worked within hospital up to band 7 or above.
  • Experience in a patient facing role.
  • Experience in using Clinical Systems.
  • Community pharmacy experience with a high level of clinical involvement and skill.
  • Primary care experience.
