Job description
Overview

An experienced Patient Pathway Co‑ordinator is sought to join the Haematology Department at King’s College Hospital NHS Foundation Trust. The role will provide efficient administration and secretarial service to Haematology consultants and multidisciplinary teams, ensuring continuity of patient care and adherence to national pathway guidelines.

Responsibilities
  • Maintain and update the Patient Tracking List (PTL) to ensure accurate 18‑week RTT pathways and Access Policy compliance.
  • Co‑ordinate patient appointments, diagnostics, inpatient stays, and pre‑operative assessments across all relevant systems.
  • Serve as the primary point of contact for the department, managing telephone calls, patient e‑mail queries, and triage of correspondence.
  • Communicate clearly, effectively, and compassionately with multidisciplinary teams, patients, families, and carers in line with the Trust’s values.
  • Manage patient correspondence via text reminder services, MyChart registration, and reference collection for clinical teams.
  • Support clinical staff inboxes, virtual clinic coordination, and clinical diary management for leave arrangements.
  • Input and edit patient data accurately in Epic, ensuring data quality, correct demographics, and escalation of duplicate records.
  • Escalate complex queries and recommend improvements to the patient care pathway and PTL processes.
  • Attend meetings, provide rota management support, and monitor internal service queues.
Qualifications
  • Excellent time‑management, planning, and judgement skills, able to work independently and under pressure.
  • Comprehensive understanding of the 18‑week RTT process, 2WW cancer pathway, and national Trust Access Policy.
  • Proficiency in Epic system usage (advanced level) and patient tracking tools.
  • Strong communication skills, both written and verbal, with attention to confidentiality and patient privacy.
  • Experience in managing telephone, e‑mail, and patient correspondence, including triage and text reminder services.
  • Knowledge of MyChart app registration and patient support resources.
  • Data quality and system quality workstream participation, ensuring standard operating procedures are followed.
