A local GP practice in Leicester is seeking a Patient Care Coordinator to work a minimum of 20 hours per week. The role involves being the first point of contact for patients and managing administrative tasks within the practice. We are looking to appoint a Patient Care Co-ordinator for a minimum of 20 hours per week to join ourbusy GP practice. This role is established as the first point of contact for patients,contractors and visitors as well as carrying out general tasks associated with a GP surgery.
You will be responsible for the general administrative and receptionduties within the Practice and work with our established processes, policiesand procedures to provide a comprehensive high quality service and dealefficiently and courteously with patient enquiries.
You will have excellent communication skills and IT literacy, a compassionate andfriendly nature who can operate in a busy environment with a range of peopleand professions, as you will be working our clinical and administration teams alongside the general public.
Your regular duties in this role will be dealing with telephone, faceto face and electronic enquiries, booking and/or amending patient appointmentsand home visits, contacting patients to provide information from the clinicalteam and assisting patients to access our services and those available in thewider community.
This role is ideal for someone who is highly organised and an effectivecommunicator who can provide a professional and warm welcome to our patients,ensuring every contact is of the highest quality.
You will be regularly processing prescription requests andproviding a full service within the Reception team.
Working part time, receivingpatients, consulting with members of practice team.
Processingpersonal and telephone requests for appointments, telephone consultations and ensuringcallers are directed to the appropriate healthcare professional.
Register newpatients, temporary residents and any requiring immediate and necessary treatment,carefully checking all details for accuracy and enter on computer registrationlink.
Taking messagesand passing on information.
Initiating contact with and responding torequests from patients, other team member and associated healthcareagencies and providers.
Processing anddistributing incoming and outgoing mail.
Filing andretrieving paperwork.
Computer data entry/data allocation andcollation; processing and recording information in accordance withpractice procedures.
Providing clerical assistance to practice staffas required, including word/data processing, filing, photocopying, andscanning.
Keeping the reception area, notice boards,leaflet etc. tidy and presentable
Cover sickness/annual leave and work reasonableovertime when required
Perform any other relevant and reasonable dutiesthat may be requested by the lead receptionist,practice manager or partners.
Undertake statutory and mandatory training asrequired.
Book/edit/cancel appointments and recallsensuring sufficient information is recorded to retrieve medical record.

We provide flexibility where required in working hours and are very staff-focussed in order for them to provide the best patient care and experience possible.

RECEPTION

Ensure that system is operational at thebeginning of each day and switched over to night service and that theanswer phone operational at the end of each day.

APPOINTMENT SYSTEM MANAGEMENT
Book/edit/cancel appointments and recallsensuring sufficient information is recorded to retrieve medical record.
Monitor effectiveness of the system and reportany problems or variations required.
Deal with homevisit requests, carefully noting all details
Arrange transportto hospital appointments for housebound patients
MEDICAL RECORDS MANAGEMENT
Retrieve and refilerecords as required, ensuring that strict alphabetical order is adheredto.
Ensure correspondence,reports, results, etc., are filed in correct record.
Ensure records are keptneat and tidy and in good repair with all necessary information recordedcorrectly on the outer cover.
Processing repeatprescriptions in accordance with practice guidelines
Handing completedrepeat prescriptions to patient and checking names and address.

CONFIDENTIALITY
Maintain confidentiality of information, acting within the terms ofthe Data Protection Act and Caldicott guidance on patient confidentiality atall times.
Maintain an awareness of the Freedom of Information Act.
Information relating to patients, carers, colleagues, other healthcareworkers or the business of the practice may only be divulged to authorised personsin accordance with the Practice policies and procedures relating to confidentialityand the protection of personal and sensitive data
