You would be joining a supportive team of qualified therapists and therapy support workers to provide senior clinical leadership to deliver quality care to our D2A patients within care homes in and around the East & North Hertfordshire area, as well as the inpatients at QVM.
You will be based at QVM in Welwyn, Hertfordshire. However, when providing the D2A service, the post holder will be required to travel between care homes within East & North Hertfordshire including Stevenage, Hatfield & Broxbourne.
QVM is a 21-bed unit located on the outskirts of Welwyn Village, Hertfordshire. QVM provides rehabilitation for patients with a wide range of conditions such as falls, frailty, stroke, orthopaedics and amputees. QVM is a purpose-built unit to also support East & North Hertfordshire's plus size pathway. Rehabilitation is provided on a generic 17‑day pathway, or a 42‑day pathway for stroke patients.
