A leading hospice organization in Stoke-on-Trent is looking for Registered Nurses to provide holistic and person-centred care to young adults facing complex health conditions. Assess, plan, implement and evaluate plans of care for service users in partnership with them, their parents/carers (where appropriate), and other professionals. Provide effective care, treatment and management within the Young Adult Unit and/or community setting. Ensure all documentation is complete and kept up to date in accordance with policy. Liaise with healthcare and social care professionals across organisations to ensure an integrated package of care and support, designed to meet the specific needs of the service user, is delivered and co-coordinated. Ensure care delivered is delivered to predetermined standards. Work in partnership to actively involve service users and their parents/carers (where appropriate) in the implementation and evaluation of care plans. Provide information for service users and/or their families as their condition progresses, ensuring they are able to make informed decisions.
