A local healthcare provider in Redcar is seeking an experienced Nurse Practitioner to join their team. The ideal candidate will be responsible for assessing, diagnosing, and treating patients within a busy GP surgery. We are looking to recruit an experienced Nurse Practitionerto support the clinical team in our busy GP Surgery in Redcar.
You will demonstrate safe clinical decision making andexpert care, working collaboratively with the general practice team to meet theneeds of patients.
The post holder will be competent in the delivery of primary care to our practice population, having direct clinical involvement and extensive clinical knowledge.
They will assess, diagnose, treat, refer or signpost patients who attend with undifferentiated or undiagnosed conditions.
They will work as an independent and autonomous Care Practitioner providing a first point of access in primary health care.
The post holder will undertake, home visits, including care homes primarily for acute conditions, as assessed by the duty doctor and within boundaries of professional qualification.
They will also be able to provide clinical supervision and support to the nursing team, and advise and support while liaising with other practice staff.
Our existing clinical team consists of 4 GP Partners, Salaried GP, Nurse Practitioners, Practice Nurses, Advanced HCAs, HCAs, Phlebotomists, Pharmacists and Pharmacy Technicians.

Job Summary
The post-holderwill be competent in the delivery of primary care to patients in the practicepopulation (with the only exception being children under the age of one),having direct clinical involvement and extensive clinical knowledge.

To undertake home visits, includingNursing/Residential Homes primarily for acute conditions, as assessed by yourduty doctor, and within the boundaries of professional qualifications.
The post holder will use advanced clinicalskills to provide education to patients, promoting self-care and empower themto make informed choices about their treatment.
They will be ableto provide clinical supervision and support to the nursing team, advice andsupport while liaising with other practice staff.
To enableclinicians to safely travel to visit patients in their own home; they areobliged to ensure that your vehicle is in good working order and covered bybusiness class car insurance (any additional premium will be paid by thepractice upon receipt of a copy of the vehicle insurance cost)
Uponrequest, a percentage of the Nurse Practitioners time may be spent on personaland practice development activities including audit, research, team developmentand clinical governance.
JobResponsibilities:
Diagnosing and treating patients
Triage and treat patientswishing to see a health care professional, making any necessary referrals toother members of the primary health care team
Acute care providing clinical support/dealing with patients bookedinto the GP On Call clinics
Domiciliary Care seeing patients in their own homes who are too ill toattend surgery and/or performing a ward round/virtual ward round at ourregistered Nursing Homes
Patients with problems needing referral tosecondary care should be discussed with the Usual GP before making suchreferral.
Ensure clinical Practice is safeand effective and remains within boundaries of competence, and to acknowledgedlimitations.
Advise patients on generalhealth care and minor ailments, with referral to other members of the primaryand secondary health care team as necessary
To undertake home visits,primarily for acute conditions, as assessed by the Duty Team, and within theboundaries of professional qualifications.
To communicate at all levelswithin the team ensuring an effective service is delivered.
To maintain accurate, up-to-date healthcarerecords appropriate to the consultation.
Ensure evidenced-based care isdelivered to the highest standards.Enhance own performance throughContinuous Professional Development, imparting own knowledge and behaviours tomeet the needs of the service.
To participate in the auditprocess, evaluation and implementing plans and Practice change in order to meetpatient need.
Contribute positively to theeffectiveness and efficiency of the team and work colleagues.
Pathological specimens and investigatory procedures
Undertake the collection of pathologicalspecimens including intravenous blood samples, swabs etc. Perform investigatoryprocedures requested by the GPs
Administration and professional responsibilities
Participate in theadministrative and professional responsibilities of the Practice team
Ensure SystmOne is kept up todate, with accurate details recorded and amended
Ensure accurate completion ofall necessary documentation associated with patient health care andregistration with the Practice
Ensure collection andmaintenance of statistical information required for regular and ad hoc reportsand audit
Attend and participate in Practicemeetings as required
Training and personal development
Training requirements will bemonitored by yearly appraisal and will be in accordance with Practicerequirements. Personal development will be encouraged and supported by the Practice.It is the individuals responsibility to remain up to date with recentdevelopments.
Participate in the education andtraining of students of all disciplines and the induction of all members of thePractice staff where appropriate
Maintain continued education byattendance at courses and study days as deemed useful or necessary forprofessional development.If it is necessary to expand therole to include additional responsibilities, full training will be given.
The post-holder will participatein any training programme implemented by the Practice as part of this employment.
Taking responsibility for owndevelopment, learning and performance and demonstrating skills and activitiesto others who are undertaking similar work.
As well as the nursing team,there is a need to work closely with Administration to ensure the smoothrunning of the Practice, reporting any problems encountered to the relevantperson.
There is also the need toestablish and maintain good liaison with other surgeries and agencies,including secondary care.

Information relating topatients, carers, colleagues, other healthcare workers or the business of the Practicemay only be divulged to authorised persons in accordance with the Practicepolicies and procedures relating to confidentiality and the protection ofpersonal and sensitive data

Health & Safety
Practice staff will comply with all policies inrelation to Health and Safety and ensure they complete any mandatory trainingas advised by the Practice Manager.Maintain an up to date knowledge of health andsafety and infection prevention & control statutory and Practiceguidelines.

Practicestaff must keep up to date with the requirements of information governance andmust follow Practice policies and procedures to ensure information is dealtwith legally, securely, efficiently and effectively. Staff must appropriatelymanage the records they create or hold during the course of their employmentwith the Practice, making the records available for sharing in a controlledmanner, subject to the statutory requirements and agreed security andconfidentiality policies, procedures and guidelines (e.g Freedom of InformationAct 2000, Caldicott guidelines). If you are required to make entries intohealth records, you must ensure these entries are legible and attributable, andthat record keeping is up-to-date.
