Overview

The Neighbourhood Nurse Lead is responsible for managing a community nursing team including the district nursing team in a neighbourhood. Provides clinical leadership, expert clinical advice and supervision to community nurses. Supports the management of patients with complex health and social needs on the district nursing caseload to enhance positive patient experience through patient pathways. Works in partnership with service users, General Practitioners, Social Care and the voluntary sector to promote a seamless service to patients. Provides proactive case management to prevent unplanned admissions to hospital; assesses, diagnosing and prescribing in partnership with General Practitioners. Advocates for patients and becomes the lead clinician for Health and Social Care integration. Responsible for patient safety and high quality care. Involves data analysis to identify patients who would benefit from case management.

Works in partnership with service users, General Practitioners, Social Care and the voluntary sector to promote a seamless service to patients. Providing proactive case management to prevent unplanned admissions to hospital for patients on the caseload. Assessing, diagnosing and prescribing in partnership with General Practitioners. Advocates for patients and becomes the lead clinician for Health and Social Care integration. Responsible for patient safety and ensuring the delivery of high quality care.

Job responsibilities

• Assesses, diagnoses, and implements treatment plans using advanced clinical skills and knowledge, collaborating with patients in treatment decisions (including planned advance directives near end of life care).
• Analyses complex information from clinical assessments to determine differential diagnosis and appropriate course of action.
• Initiates therapeutic interventions aligned with the best available evidence to improve health outcomes.
• Recognises and manages early symptoms of disease exacerbation and acute illness using current practice standards.
• Advocates for patients and communicates with GPs and hospital consultants to ensure robust clinical care.
• Designs and monitors care plans for patients managed by integrated community teams and other specialists as appropriate.
• Empowers carers and family members as equal partners in care.
• Makes referrals to other specialists where indicated.
• Independently prescribes in accordance with relevant guidelines.
• Embraces and uses technology such as telehealth and telecare to enhance patient outcomes.
• Delivering High Quality Care and Measuring Impact – Leads on implementing national nursing strategy and using the 6Cs as a basis for care; leads patient safety and safeguarding in collaboration with Social Care; conducts nursing care audits; promotes Trust values; facilitates multidisciplinary supervision; evaluates practice against evidence; reports data for dashboards.
• Maintains accurate and contemporaneous patient records in line with Trust and NMC standards; records communications with other professionals; develops outcome-focused care plans with clear reviews; ensures records quality across the team.

Qualifications and Training

Essential

• First level registered nurse with current Nursing and Midwifery registration
• Qualified at degree level with Community Nursing Sister Specialist Practice or Advanced Nurse Practitioner
• Non-medical Independent Prescribing qualification or working towards
• ENB 998 Teaching and Assessing in Clinical Practice or Mentorship Course or working towards
• Recognised leadership and development training or equivalent experience

• MSc in Health Care
• Management qualification

• Experience of leading and managing skill-mixed nursing teams in the community
• Collaborative working with multi-professionals and negotiating care pathways across organisational boundaries
• Experience of working autonomously and managing people
