Newly Qualified Nurse Care Manager / ASYE Social Worker (SO1): £34,434 – £36,363 Nurse Care Manager (PO1) / Social Worker (PO1) (minimum 1 year post‑qualifying experience): £38,220 – £40,777 Additional grades: £42,839 – £46,142 (depending on experience)

Overview

As a Nurse Care Manager or Social Worker you thrive on enabling adults with learning disabilities to realise their potential and live life fully. You will deliver a person‑centred, holistic approach, working closely with carers, families and a wider community to provide high‑quality support and strengthen the partnership between health and social care services in Leeds.

About You
  • Registered with the Nursing and Midwifery Council as a Learning Disability Nurse (or a similar nursing qualification with substantial learning disability experience) or Social Worker (registered with Social Work England) on commencement.
  • Knowledge and understanding of current learning disability practice and multidisciplinary team roles, including statutory and voluntary agencies.
  • Skills in effective assessment, planning, intervention and evaluation of work in supporting people with learning disabilities.
  • Ability to build good working relationships with professional colleagues and partners, and engage positively with individuals, families and carers.
  • Experience using a person‑centred, holistic approach and creativity in problem solving and reflective practice techniques.

Responsibilities
  • Manage an allocated caseload and regularly monitor, review and evaluate changes to ensure effective service delivery.
  • Draft and implement service and support plans based on assessed needs and risk, in partnership with individuals, families and carers.
  • Analyse information to support the ongoing needs of service users and their families.
  • Contribute to the review of plans relating to assessed needs and services.
  • Work within the Safeguarding Framework to investigate and manage risks where individuals have experienced or may experience significant harm.
  • Prepare concise, informative reports and other documentation for legal and operational purposes.
  • Maintain accurate case notes and records using management information systems.
  • Develop and maintain effective working relationships with internal and external professionals to ensure integrated and holistic care.
  • Participate in meetings and relevant forums, manage priorities and ensure statutory legislation is met.
  • Engage in professional development, comply with governance and policy, and contribute to safeguarding practice.
  • Support individuals and families in decision making, resolving conflicts, and promoting independence.
  • Assist with the development of safeguarding plans and the commissioning of services to minimise risk.
  • Work collaboratively with other agencies, chair meetings where appropriate, and support new referrals.
  • Under instruction, undertake safeguarding investigations and related assessments as required.
  • Support the implementation of all relevant practice standards, policies and procedures.
  • Plan and negotiate appropriate services and interventions within delegated authority.
  • Undertake Care Act, Mental Capacity Act and other statutory assessments as aligns with professional capability frameworks.
Qualifications and Experience
  • Registered LD Nurse or dual LD Nurse / Social Worker qualification; registration with the Nursing and Midwifery Council or Social Work England must be held upon commencement and maintained throughout employment.
  • Demonstrated ability to plan, manage, prioritise and evaluate work with users.
  • Strong communication skills, able to produce accurate records, reports and professional documentation.
  • Proficient in relevant IT systems, including Microsoft Office, internet and email.
  • Knowledge of relevant legislation, guidance, policies and experience applying them consistently.
  • Experience in applying a person‑centred approach and recognising the diverse needs of service users and families.
  • Experience in safeguarding, risk assessment, and commissioning of services.
  • Experience of advocating, user involvement and reflective practice techniques.
  • Minimum 12 months post‑qualification experience working in a statutory or health setting.
