Buckinghamshire Healthcare NHS Trust is seeking a dedicated MND Care Co‑ordinator to join our team. This is a unique opportunity to provide expert care coordination across the Trust, working closely with neurologists, palliative care teams, the Oxford MND Care Centre, and the MND Association.

We’re looking for a registered healthcare professional (e.g. Nurse, AHP) with:

  • Significant experience in neurological or palliative care.
  • Advanced communication and leadership skills.
  • A commitment to holistic, patient‑centred care.
  • The ability to work across settings including homes, hospitals, and care homes.
  • A proactive, innovative approach to service development.

  • Registered healthcare professional (e.g. Nurse, Allied Health Professional) with current professional registration
  • Evidence of continued professional development

  • Postgraduate qualification in palliative care, neurology, or related field
Experience

  • Significant experience working with patients with complex neurological conditions
  • Experience in care coordination across multidisciplinary teams
  • Experience in service development and audit
  • Experience working with MND patients
  • Experience in leading MDTs and clinics

Desirable criteria

  • Experience in research or publication
